Natural Anti-Inflammatories: What Does the Evidence Really Show?
For years now, patients have been asking me an important question: beyond my prescriptions, what else can I do to reduce inflammation?
It is a reasonable question. Our understanding of inflammation, the microbiome, and the ways diet and lifestyle influence immune activity continues to evolve. Modern rheumatology can incorporate these approaches, but they should complement—not replace—proven disease-modifying medications.
So, which natural anti-inflammatories are actually worth discussing?
Do omega-3 fatty acids help reduce inflammation?
Yes, omega-3 fatty acids are among the better-studied dietary approaches for inflammation, with the strongest evidence in rheumatoid arthritis.
Omega-3s, particularly EPA and DHA, influence inflammatory signaling and give rise to specialized molecules that help resolve inflammation. In rheumatoid arthritis, pooled trials show modest improvements in joint pain, tenderness, and function, generally with animal-source omega-3s at doses above 2 grams per day taken for three to six months (Sigaux et al., Arthritis Research & Therapy, 2022).
The evidence is less convincing in axial spondyloarthritis. A systematic review found no clear benefit, although one small ankylosing spondylitis trial suggested improvement at higher doses (Van den Bruel et al., Frontiers in Medicine, 2025).
For people with inflammatory bowel disease, omega-3s are generally well tolerated but have not been shown to maintain remission and should not be considered IBD therapy (Cheifetz et al., Gastroenterology, 2017).
What should I know before taking fish oil?
Choose a product that clearly lists its EPA and DHA content. Some fish-oil and cod-liver-oil products also contain vitamin D or calcium, which can be important for people with calcium-regulation disorders. Taking fish oil with food may improve tolerability, and enteric-coated capsules can sometimes reduce burping and reflux.
Does turmeric or curcumin help arthritis?
Curcumin, the active compound in turmeric, has shown promising but still preliminary evidence for reducing pain and inflammation in several rheumatic conditions.
Curcumin influences several inflammatory pathways, including NF-κB. In rheumatoid arthritis, multiple meta-analyses have reported improvements in disease activity scores, inflammatory markers such as ESR and CRP, and tender joint counts (Fan et al., Frontiers in Immunology, 2025; Zhang and Niu, Medicine, 2025).
A broader arthritis meta-analysis including rheumatoid arthritis, osteoarthritis, and ankylosing spondylitis found improvements in pain and inflammation with doses ranging from approximately 120 to 1500 mg daily (Zeng et al., Frontiers in Immunology, 2022).
However, most studies are small and the certainty of evidence is low. Curcumin should therefore be viewed as a potential adjunct, not a substitute for established arthritis treatment.
Curcumin has also been studied in ulcerative colitis and may have potential benefits for people with overlapping gastrointestinal and joint inflammation (Rossi et al., European Journal of Gastroenterology & Hepatology, 2016).
Does curcumin have side effects?
Curcumin is generally well tolerated, although higher doses can cause gastrointestinal symptoms. It is poorly absorbed on its own, so many formulations include piperine, or black pepper extract, to increase absorption.
Curcumin and other supplements can also interact with medications, so patients taking blood thinners or other medications should discuss supplementation with their physician.
Is ginger an anti-inflammatory?
Ginger has anti-inflammatory properties and may provide modest benefits for pain and inflammation, although the clinical evidence is more limited than for conventional anti-inflammatory medications.
Ginger influences inflammatory pathways including COX, LOX, and NF-κB. One randomized trial in active rheumatoid arthritis found that 1500 mg of ginger daily for 12 weeks favorably affected immune-regulating gene expression and reduced disease activity (Aryaeian et al., Gene, 2019).
Systematic reviews suggest modest pain-relieving and anti-inflammatory effects across arthritis and other rheumatic conditions. Ginger has also been studied for its potential anti-inflammatory effects in inflammatory bowel disease (Dzwonkowski et al., Current Gastroenterology Reports, 2025).
Can ginger increase bleeding risk?
At culinary doses, ginger is generally considered safe. At supplement-level doses, however, it may affect platelet function and could potentially increase bleeding risk, particularly in people taking blood thinners. It can also cause heartburn or gastrointestinal discomfort.
What other natural anti-inflammatories have been studied?
Several other supplements have been studied, including Boswellia, quercetin, green tea, resveratrol, and probiotics. However, the evidence is more limited and less consistent than for the supplements discussed above.
Vitamin D is somewhat different. Correcting vitamin D deficiency is important for bone health and may have broader health benefits, but vitamin D should primarily be viewed as correcting a nutritional deficiency rather than as an arthritis treatment.
Can turmeric, ginger, or fish oil increase bleeding risk?
Some supplements, particularly turmeric, ginger, and fish oil, may modestly affect bleeding risk. This is especially important for people taking blood thinners or those preparing for surgery.
Patients should discuss supplement use with their physician before starting these products, particularly if they take anticoagulants or antiplatelet medications.
Can turmeric affect the liver or interact with methotrexate?
Turmeric and curcumin supplements have been associated with rare cases of liver injury. Because methotrexate can also affect the liver and requires routine liver monitoring, patients taking methotrexate should discuss curcumin supplementation with their rheumatologist.
Is exercise more important than supplements for arthritis?
Yes. Exercise has stronger evidence than any supplement as an integrative intervention for improving function in rheumatoid arthritis and spondyloarthritis (Lin et al., Frontiers in Medicine, 2023).
A Mediterranean-style dietary pattern rich in vegetables, fruit, whole grains, legumes, olive oil, and fish may also improve disease activity and quality of life in rheumatoid arthritis (Van den Bruel et al., Frontiers in Medicine, 2025).
Practices such as yoga and tai chi may provide additional benefits for pain and function.
What does the American College of Rheumatology say about supplements?
The American College of Rheumatology's 2022 guideline provides important perspective on diet, exercise, and supplements in rheumatoid arthritis (England et al., Arthritis Care & Research, 2023). These are specifically the 2022 American College of Rheumatology Guideline for Exercise, Rehabilitation, Diet, and Additional Integrative Interventions for Rheumatoid Arthritis.
The guideline:
Strongly recommends consistent engagement in exercise.
Conditionally recommends a Mediterranean-style diet.
Conditionally recommends against dietary supplements for treating rheumatoid arthritis because evidence has not demonstrated consistent, meaningful benefits for pain, physical function, or disease activity.
Recognizes that vitamin D may have a role in bone health and fish oil may have cardiovascular benefits.
In other words, the ACR does not recommend relying on supplements to treat RA.
Are natural anti-inflammatories safe?
“Natural” does not necessarily mean risk-free. Supplements are not regulated in the same way as prescription medications, and product quality, purity, and dosing can vary.
Botanical supplements can interact with medications, and combination products may contain additional ingredients that are not appropriate for everyone.
Anyone taking immunosuppressive medications or blood thinners, or anyone with kidney, liver, gastrointestinal, or calcium-related conditions, should discuss supplements with their healthcare team before starting them.
So, should I take natural anti-inflammatory supplements?
Possibly, but selectively. Some supplements have promising or modest evidence, but none should replace proven rheumatologic treatment.
For most patients, the strongest foundation remains regular movement, a nutritious dietary pattern, appropriate medical treatment, and attention to overall health. Supplements can be considered when there is a reasonable evidence base, a clear potential benefit, and an acceptable safety profile.
